Print this form, fill it out, and place it in an envelope with SAR written in bold letters across it. Leave the envelope in your vehicle when parked at a trail head or other wilderness access/boating area for a hiking/camping/hunting trip. Make sure the envelope is in plain view to someone looking in from the outside, like on the seat or dashboard. The information is intended to help search and rescue should you become overdue.
